Providing services in San Luis Obispo County to families needing help in maintaining safe, healthy and stable environments.

The goal of the Health Navigator Program is to keep children 0-5 healthy and connected to healthcare by linking families with health-related resources. Health Navigators are able to assist families in understanding and enrolling with insurance programs for which they may be eligible, and connecting with affordable health care services.

The Health Navigator program is funded through a grant from First 5 SLO. The Health Navigators will work closely with families, advocates and community partners to ensure that children 0-5 years of age are connected with resources.

Health Navigators will work with families with children prenatal to age 5 years old that are impacted by low or no insurance coverage by offering case management services and referrals to local agencies. In addition to working with families, the Health Navigator will be a resource to child care providers, early childhood education programs, schools and the community.
